How can we know that our relative is being cared for in the way he/she needs?

In order to clarify this type of doubt, at Matia Fundazioa we offer the home assessment and monitoring service.

With this service we aim to provide support to the family and guarantee that the attention and care that the person needs is provided and carried out in an adequate manner.

To this end, a social worker with experience in our model of care will be in charge:

• Carry out a comprehensive assessment at home.
• Drawing up and agreeing a personalised Care Plan.
• Monitoring of the Care Plan.

In this way we guarantee that the services are adapted to the needs of the person and their environment.
